Nagavalli is a supernatural thriller; its story revolves around characters living in a house in Tirupati. After a family member receives a Nagavalli portrait as a prize in the Nagavalli Dance competition, various characters start having abnormal experiences. In order to save themselves and unfold the mystery, the family looks to Ramachandra Siddhanthi and Dr. Vijay. The story starts off with an ancient painting of Chandramukhi alias Nagavalli (Anushka Shetty), distributed as a prize to bharathanatyam dancer Gayathri (Kamalinee Mukherjee), her husband and her family. Gayathri lives in a palatial house in Tirupathi with her husband, father Shankar Rao (Sharat Babu), mother Parvathi (Prabha), sisters Geetha (Shraddha Das) and Gowri (Richa Gangopadhyay), maternal uncle Appa Rao(Dharmavarapu Subramanyam), his wife and maternal cousins Pooja(Poonam Kaur) and Hema(Suja) happily.

As a youngster, Chotu is abducted by Chidambaram -a man who uses a gang of beggar children to his own advantage. Chotu eventually betrays his cruel master to the police, and receives a small reward. Years pass and Chotu now works as a bounty hunter. He frequently visits the imprisoned Chidambaram, hoping that he will reveal some clue to the identity of his real family. Chotu life is further complicated when he meets Sameera -a beautiful guitar teacher and sister to one of the criminals he is after, and Johnny Bhai -the sadistic goon at the heart of an assassination plot.

Sanjay Sahu (Pawan Kalyan) hails from a poor rural family at remote village in Karimnagar. His brother dies of heart disease as a child. His father commits suicide as crops fail and his mother too is no more. While going to burial ground, he repeatedly slips and swoons at a stone pillar. He plucks it out, which enrages the henchmen of a land-grabber Damodar Reddy (Mukesh Rishi). When they attack him, Sahu beats them to pulp, and disappears into a forest and joins the naxalites. During a combing operation, Sahu and a police officer (Prakash Raj) meet each other and become good companions under strange circumstances. Sahu comes to Hyderabad and duly rehabilitated as a college student.
